Data Collection Form

The Department of Psychology gathers information after a job or affiliate offer to comply with policies and laws and help you get started. This information is essential for verifying credentials, conducting background checks, and managing employee records for benefits and payroll. The data collection form is also used to gather information for anyone not affiliated with the University of Maryland on a dissertation committee without conducting background checks.

Before starting, have the following ready:

  1. A copy of your unofficial transcript1 2 if you have a bachelor’s degree or higher.
  2. A signed and dated CV if you have a bachelor’s degree or higher.
  3. If you are a non-U.S. citizen1, you will need a copy of your work authorization documents.

1 Special note to Dissertation Committee Members who are not affiliated with the University of Maryland:
Select “Dissertation Committee Member” for the Highest Degree. This will bypass the need to upload your transcript and/or work authorization if you are a non-U.S. Citizen. All other information will be required.

2 Special note to Affiliate Members
Select “Affiliation Member” for the Highest Degree. This will bypass the need to upload your transcript. All other information will be required.
